Survey Question Types differ from Test Question Types in that there are no correct or incorrect answers. These question types are not scored; rather, they are used to collect feedback. You may include survey questions in an assessment that contains test questions, although if you do, you may want to place them in their own question group in order to separate them from the rest of the assessment.

Choose One
A Choose One question allows you to present up to 50 answer choices, from which respondents select only one by clicking one of the radio buttons to the left of the answers.
Choices: In each text area you wish to use, you must use at least two. Enter the answer choices you would like to present for the question. Fields for additional answer choices are available if needed.
Dropdown Choose
A Dropdown Choose question allows you to present a list of answer items in a dropdown format. Respondents select one option from the list.
Choices: Enter each answer on its own line.
IMPORTANT: It is recommended that answer choices be kept reasonably short for proper display within the dropdown.
Agree / Disagree
This Likert scale question type allows respondents to indicate their level of agreement with a statement.
Choices: Response choices are controlled at the account level.
Excellent / Poor
This question type allows respondents to rate a statement along a scale from Poor to Excellent.
Choices: Can be configured at the workspace level.
Stackable Numeric Rating
This question type groups similar rating questions together in a matrix-style layout for easier comparison and response.
Respondents select a rating for each question independently.
Stackable Dual Numeric Rating
This question type allows respondents to provide two ratings per question using separate rating headers.
Each question requires two independent ratings.
Net Promoter®
This question type allows respondents to rate likelihood on a scale from Not At All Likely to Extremely Likely.
Choices: Controlled at the account level with optional numeric ordering.
All That Apply
Respondents may select multiple answers from a list of options.
Choices: Provide at least two options. Additional options can be added as needed.
Shuffle Answers: Optionally randomizes answer order.
Matrix
A Matrix question allows you to create a grid of rows and columns where respondents select one choice per row.
Column Labels: Define available responses (minimum of two).
Row Labels: Define individual items or questions to be rated.
Short Answer
Allows respondents to enter a brief free-form response.
Opinion
Allows respondents to provide a longer, open-ended response with no limit on length.
